Agreement — Unique Creative Studio

ORDER ACKNOWLEDGEMENT

By submitting this order form, I hereby consent to Unique Creative Studio communicating  and I acknowledge and agree to the following:

 I am authorised to place this order on behalf of the business, organisation or group.

 I have the consent to provide the names and details of staff members or group for inclusion in the custom song.

 I understand that Unique Creative Studio provides an audio song only and is not responsible for any recording, filming or sharing of the song.

I understand that content is created to be positive and for businesses, workplace-appropriate and may be adjusted if required.

I confirm that all fun facts shared are appropriate and approved by the staff members or group.

I have read and agree to the Terms and Conditions, Additional Terms and Conditions and all important Information on our website.

I acknowledge the Revision Policy in that all revision requests must be submitted in one consolidated response per round.  Your custom song includes one round of revisions. Revisions may include word changes, tone adjustments, or emphasis on specific names or moments and are to be submitted in writing via email to admin@uniquecreative.studio, following which the song will be submitted with requested changes, and the order is final.  Additional revision rounds are available at $150 AUD per round, subject to feasibility.
